左心系IEの経口抗菌薬へのスイッチ

Partial Oral versus Intravenous Antibiotic Treatment of Endocarditis

P：状態が安定した左心系IE 

I：経口抗菌薬に変更

C：静注抗菌薬を継続

O：全死亡率、心臓手術、塞栓症、菌血症の再燃

原因菌はレンサ球菌、Enterococcus faecalis、黄色ブドウ球菌、CNS

最低10日間治療し、2群に割り付け

非劣勢基準を満たした

アロプリノールのCKDへの効果

O：eGFRの変化

アロプリノールがCKDの進行を抑制させるかを調べたRCTで、有意な差は認められなかった。

消化管出血におけるトラネキサム酸の使用

I：24時間トラネキサム酸投与

C：プラセボ投与

O：5日以内の出家死

Primary outcomeではトラネキサム酸群で3.7％、プラセボ群で3.8％。有意差は認めなかった。

合併症やトラネキサム群でVTE、seizureのリスクが高かった。

P：安定狭心症

I：PCIとoptimal medical therapy

C：optimal medical therapy alone

O：死亡率　nonfatal myocardial infarction

安定狭心症に関するPCIの予後改善効果を調べたRCT。

Entry criteria

1カ所以上の近位冠動脈狭窄>70%

虚血の客観的証拠がある

80%以上の狭窄を有し狭心症上がある

Exclusion criteria

CCS class Ⅳ

負荷試験の明らかな陽性

治療抵抗性の心不全、心源性ショック

EF30％以下

6ヶ月以内に再灌流治療を受けている

PCI不適病変

Primary outcomeに有意差はなく、secondary outcomeの死亡+MI +脳卒中、ACSによる入院も有意差はなかった。

I：デキサメタゾン投与

O：9ヶ月間の死亡または重度障害

ベトナムで実施されたRCT。

デキサメタゾン群で死亡率の低下が見られ、重度障害に関しては有意差は認められなかった。また、死亡あるいは重度障害でも有意差は認められなかった。
